INDEPENDENT ASSESSMENTS FOR ADULTS AND OLDER PEOPLE. COMBINED WITH LEGAL ASSISTANCE TO GET WHAT YOU WANT.

We provide independent assessments for people who wish to fund their own home care services or residential/nursing care.
If you find it difficult to cope with the day to day tasks and need help in your own home we aim to provide a holistic assessment that will help you to have greater control over the type of support or help that they need.

The Assessment Process

This will be achieved by involving you in the assessment process as well as gathering information from family members, carer’s and other professionals who know your medical history (such as your doctor), as well as taking into consideration your age, disability, gender, cultural background, religious beliefs, ethnic origin and sexual orientation.
We will concentrate on specific areas in order to maximise your independence. These include:-

Care Plan

Once we have collated all the relevant information we will tailor your care plan to outline the type and level of care that your need. Whilst considering the most cost effective options available.

Review and Monitoring

We will review your care plan within the first 3 months to make sure that you are happy with the quality of care that you are receiving and then on a yearly basis.

Reassessments

If you feel that you need more help with your personal care and daily living tasks we will visit you to undertake a reassessment and amend your care plan to reflect an increase in the level of care that you need.

Confidentiality Policy

We ensure that your personal information is treated with the strictest confidence and will not be shared without your written consent.

Hours of Work

We are available 6 days week Monday – Friday from 9.30am – 5.30pm and Saturday from 10.00am – 2.00pm.
